Cosmetic Concern:
33-year old woman with heavy upper eyelids and lower eyelid fullness.

Diagnosis:
Heavy brows are a feature she as had since childhood.  The lower eyelids demonstrate a slight fullness from fat.

Treatment:
Upper and lower blepharoplasty.  The goal here was to remove enough skin in the upper eyelids to accentuate the upper eyelid platforms without removing too much of the fullness of the upper eyelid which is an important feature of her face. Her mother had a bad upper eyelid surgery and at first her mom was critical of my work because she thought I had not removed enough eyelid tissues.  Once that patient healed her mother cried and apologized for being critical.  The patient herself is thrilled with the result and has new confidence.  She told us that this surgery literally changed her life.

Cosmetic Concern: 
28-year-old woman concerned with eyelid symmetry.  In talking with her, her primary concern is that the eyes are both heavy but the right eye look heavier that the left eye.

Diagnosis:
Bilateral upper eyelid ptosis with facial asymmetry.  Note that her right eye sits lower in the face than the left globe.  Recognizing this fact determines the limits for what will be possible surgically.

Treatment:
  1. Bilateral upper anchor blepharoplasty.
  2. Bilateral upper eyelid ptosis repair.

Discussion:
It is essential to correctly diagnose the nature of the anatomic issues.  The punishment should fit the crime, meaning that surgery must be appropriately tailored to the individual situation.  Her result is completely natural, yet it brightens her eyes.  She was thrilled with her result.

Cosmetic Concern:
54-year old man who is interested in looking younger and less tired.

Diagnosis:
  1. Upper eyelid ptosis.
  2. Hollow under eyes and dark circles.
Treatment:
  1. Bilateral upper eyelid ptosis.
  2. Upper eyelid blepharoplasty, skin only.
  3. Under eye fillers

Discussion:
It is critical that men not have all of their features removed by cosmetic surgery.  Less is more. In particular, it is important to leave a little skin in the upper eyelid so that the eyes are not feminized.  He had about 1 week out of the office but was able to conduct business by phone the day after his surgery.
