Asian Eyelid Procedure Overview
Procedure: Facial Rejuvenation
Cosmetic Concern:
49-year-old woman looking to rejuvenate her appearance. Her concerns were her eyes and lower face. She elected to have eyelid surgery and lower face and neck liposuction.
Diagnosis:
- Hooding of the upper eyelids
- Herniated lower eyelid fat
- Fullness of the lower face and neck
Treatment:
- Upper and lower eyelid surgery
- Lower face and neck liposuction
Discussion:
Subtle but natural surgery has refreshed her appearance. The upper eyelid surgery preserves her upper eyelid fold but allows her to hold make-up on her eyelid platform. The lower eyelid surgery was accomplished with a transconjunctival approach without a skin incision. The lower face and neck liposuction further compliments her facial features.
Procedure: Facial Rejuvenation
Cosmetic Concern:
29-year-old woman with prior unsatisfactory eyelid surgery. She feels too much of the upper eyelid folds were removed, Westernizing her upper eyelids. After photograph shows her after Dr. Steinsapir has reconstructed her eyelids.
Diagnosis:
- Over-aggressive prior blepharoplasty
- Uncorrected upper eyelid ptosis
Treatment:
- Upper eyelid ptosis surgery
Discussion:
By correcting the upper eyelid ptosis, the excessive amount of exposed upper eyelid platform was reduced placing the upper eyelid folds in better balance with the upper eyelid folds. Although it is very difficult to increase the size of the upper eyelid fold, the presence of upper eyelid ptosis created the opportunity to restore upper eyelid harmony.

Cosmetic Concern:
47-year-old woman with a prior history of unsatisfactory upper blepharoplasty. She did not like the complex double folds that she was left with as a result of her prior surgery. She also felt her upper eyelid folds were heavy. She requested revisional upper eyelid surgery. The after photo shows her following this surgery.
Procedure: Asian (Double Fold) Eyelid Surgery
Cosmetic Concern: 32-year-old woman desires a double eyelid fold
Diagnosis
- She has a rudimentary crease in the upper eyelids. This leaves her with no place to apply eyeliner on the upper eyelid.
Treatment:
Asian eyelid surgery is not about westernizing the eyelid. Rather it is about conserving normal eyelid structure while at the same time providing complementary eyelid definition to produce the desired result. Typically this type of surgery involves about a week of downtime.
Procedure: Fullness in the Lower Eyelids
Cosmetic Concern: 59-year-old man bothered by the fullness in the lower eyelids.
Diagnosis
- Herniated lower eyelid fat.
- Lower eyelid wrinkles.
- Sun damaged skin.
Treatment:
- Transconjunctival lower eyelid surgery.
- Lower eyelid chemical peel.
Discussion:
Lower eyelid surgery reduced the fullness of the lower eyelid by removing eyelid fat. This tends to deflate the lower eyelid and slightly increase the lower eyelid wrinkles. The chemical peel performed at the time of the lower eyelid surgery, helps to firm the lower eyelid skin and reduce this tendency and avoids the need for surgery to remove the excess lower eyelid skin.

Cosmetic Concern: 74-year-old woman with left facial weakness following Bell’s palsy.

- Left forehead paralysis
- Pseudoherniated orbtial fat
Treatment:
- Endoscopic forehead lift
- Upper eyelid surgery.
- Lower eyelid surgery.
Procedure: Revisional Asian Eyelid Surgery
Cosmetic Concern:
28-year-old with a history of prior Asian blepharoplasty. She was unhappy with the absence of her double fold, poorly defined upper eyelid crease and crepe-like upper eyelid skin.
Diagnosis
- Excessive removal of upper eyelid skin
- Upper eyelash ptosis
- Poorly defined upper eyelid crease
Treatment:
Micro-anchor blepharoplasty. Note her appearance at one week with minimal bruising. In the after photograph she is 6 months out from surgery with a significant improvement in the appearance of her eyes with a well define double fold.
